JERVIS DAVID Passed away peacefully at home on August 25th 2016 aged 67 years. Loving husband to Barbara. Dear dad to Marcus and his wife Nicola. Service to be held at Kirkley chapel on September 16th 2016 at 11.00am followed by interment at Kirkley Cemetery. Rock music tee shirts welcome but not essential. No floral tributes, but donations if desired for British Heart Foundation and Help for Heroes directly to the charities.
